Abstract
APST is used as a stress test to measure the dynamic response of the heart. It is considered to reflect the myocardial ″reserve capacity″ . Extensive intra- and interpatient variability may occur. An objective method to cope with this variability was assessed in the APST data of 44 normals and 71 patients with coronary artery disease (CAD) and compromised myocardial function. Reproducibility was tested with duplicate determinations in 10 CAD patients. Within a heart rate trajectory of 70 to 140 beats per minute, the relation between heart rate and LV pressure derived variables can be regarded as linear, both in normals and in patients with CAD. Results are reduced to measures of offset and slope. A further data reduction is carried out with linear multivariate discriminant analysis. The results are applicable as a scaled factor, indicating the extent of myocardial damage.
