Smart patients: Holistic empowerment of patients to become experts in their own health

Smart Patients is a two year, Erasmus + funded project that has developed online modules to facilitate and support citizens in promoting and managing their health. There are three modules Prevent-Empower-Participate, which are each divided into four or five submodules and, which cover topics such as nutrition, work life balance, mental health, cultural awareness, patient skills, building relationships and reactions to illness. Submodules have short quizzes to facilitate engagement and all modules are currently available in English - https: smart-patients.eu main toolbox.php By the end of summer 2018, the modules will also be available in Arabic, Pashtun, German, Greek, Portuguese, Croatian, Slovenian and Slovak. To support anywhere access the materials can be accessed via both pcs and smartphones.
